In order to keep statement and billing costs to a
minimum, we ask that you pay for all services and
treatments at the time they are rendered. Co-Pays
are expected at the time of service.
Visa, Mastercard and checks are accepted for your
convenience.
Payment plans are available upon request.
We are available to prepare and submit insurance
claim formats to your insurance company.
Any unpaid portion of your bill not covered by insurance
benefits is due within 30 days following the final
insurance payment. Ultimate financial responsibility
for payment rests with the patient or his/her family.
